Transitioning home from the hospital after an injury, accident, surgery, or other health condition can be a very stressful time. HealthFlex transitional care professionals help relieve your anxiety while ensuring a safe trip home.
All HealthFlex Transitional Care Coaches (TCCs) are specially trained to fully educate you and your family, and to answer any questions and address any concerns you may have about returning home. We work closely with the hospital, your primary doctor, and you and your family to customize a plan that provides all the support when you need it most.

- Personal coaching to address patient and family concerns
- Durable medical equipment (DME) coordination and loaner program
- Transportation coordination from hospital to home (with wheelchair access)
