The Advanced Care offered by HealthFlex enable our registered nurses and clinicians to more effectively manage our patients’ daily need––in the comfort of their homes.

This lets us better control patient outcomes, helps patients retain their dignity, and most importantly, lets patients keep their independence as long as possible. This is especially true when rehabbing after surgery, managing a chronic disease, or educating a family about life-limiting situations.

The HealthFlex Diabetes Management and Care Program is designed to emphasize the education, monitoring, and treatment of patients with Type I and Type II diabetes, Diabetes with Polyneuropathy, and Diabetic Retinopathy.
Fall Prevention & Safety
In this program, prevention tips are provided at each visit and we perform in-home assessments to ensure there are no safety hazards.
