HealthFlex CHF Services
- Assess patients’ and families’ understanding of the disease and its management
- Assess the current treatment plan, review the medication regimen, and develop a compliance plan
- Collaborate with the primary doctor regarding the treatment plan
- Perform a nutritional assessment by registered dietitian
- Assess lifestyle practices including physical abilities and limitations plus the ability to perform activities of daily living
- Create a home exercise program and patient energy conservation plan
- Develop an individualized daily routine for monitoring weight, heart rate, oxygen saturation, blood pressure, and peak expiratory flow, as specified by the patient’s doctor
