The HealthFlex Congestive Heart Failure (CHF) Management Program empowers and supports each patient and family to manage the disease. The program integrates skilled nursing, physical therapy, occupational therapy, and registered nutritionists.
Home care options for pulmonary management
If you’re one of approximately five million Americans affected by CHF annually, our HealthFlex CHF Management Program includes a lifestyle change with skilled nursing for evaluation and management of symptoms, home health aides, nutritionists and dieticians, physical therapy, and occupational therapy.
HealthFlex CHF Services
- Assess patients’ and families’ understanding of the disease and its management
- Assess the current treatment plan, review the medication regimen, and develop a compliance plan
- Collaborate with the primary doctor regarding the treatment plan
- Perform a nutritional assessment by registered dietitian
- Assess lifestyle practices including physical abilities and limitations plus the ability to perform activities of daily living
- Create a home exercise program and patient energy conservation plan
- Develop an individualized daily routine for monitoring weight, heart rate, oxygen saturation, blood pressure, and peak expiratory flow, as specified by the patient’s doctor

Hypertension Care
At HealthFlex, we offer an interdisciplinary home care program that can help our patients manage their hypertension and even improve their symptoms over time.
Our services are delivered by registered nurses, registered dietitians, occupational and physical therapists, and home health aides. The HealthFlex Hypertension Care Program helps patients achieve heart-related as well as general healthcare goals to minimize the future need for urgent or emergency care.

- Educate patients and caregivers how to monitor blood pressure, keep records, and manage medications
- Create a meal plan that ensures each patient is on a balanced diet
- Assist patients in getting the daily physical activity needed to maintain a healthy weight and active lifestyle
